PaRappa, a sentient cartoon dog, can (in most versions of the game) play in a level that takes place in Hell, implying that he died to get there, though unlike the game's other protagonist, there is not a cutscene that shows him dying beforehand.
Lammy, a lamb, dies and goes to Hell in some versions of the game, though she comes back to life shortly after. It should be mentioned that Lammy is of roughly human sentience, so whether she counts as an "animal" for the purposes of this question is up for debatable.
Arguably. There's a scene where Lammy eats too much pizza, which people often believe is her getting fat. Later on her stomach becomes small when entering the baby delivery hospital/nursery.
The midwife cattepillar sees Lammy bloated from eating too much pizza and assumes she's pregnant, as a march of pregnant rabbits lead her to the baby delivery hospital. Later on it's seen that all of these babies are delivered, but the progress isn't shown.
Nearly. In Lammy's dream, the bullies from Parappa the Rapper decide to pick on and harass Milkcan, to which Lammy beats them up by getting handed her guitar.
Arguably. Stage 4 has Captain Fussenpepper, who switches between angry drill sargeant and cowardly pilot whenever he gets hit on the head. Neither alter is evil (though the drill sargeant one isn't the nicest guy ever), but I'm saying this as a singlet. So take it with a pinch of salt, ig.
This only happens if you fail stage 4, and nobody is shown to be injured or killed by it. That being saod, if you start doing bad or awful, the plane starts nosediving and only corrects itself when you start playing better.
No, but fail Stage 5 (with the AWFUL rating), and Teriyaki grows to be 50 something feet tall (causing a hole in the audience pit) before berating Lammy for her failiure.
Lammy says "st*pid" in one of the versions of the game, which is a commonly used derogatory term. The pilot ever so slightly depicts stereotypes of PTSD from war (With, when his head is hit by the control panel above, he goes from mellow to raging in a matter of seconds and makes Lammy fly the plane, assuming she's a pilot.) Otherwise, the game is clean of this.
