Between the events of 'Saw' and 'Saw II', a sick and desperate John Kramer travels to Mexico for a risky and experimental medical procedure in hopes of a miracle cure for his cancer, only to discover the entire operation is a scam to defraud the most vulnerable. Armed with a newfound purpose, the infamous serial killer returns to his work, turning the tables on the con artists in his signature visceral way through devious, deranged, and ingenious traps.

This is getting into spoilers, but the protagonist tries to purchase a gift for one of the characters, only to find that they have tricked him and skipped town. It is a very sad scene, and I found it emotional.
Many people are stalked by someone a pig mask, then they are snuck up on and zapped with a cattle prod or knocked out with a blow on the head. They wake up in a much worse situation
Two cases. First is Amanda, who was a Jigsaw victim in an earlier movie but has now joined him and helps him torture others. Secondly is Cecilia who, without spoiling too much, escapes from a death trap and instead forces John Kramer to play his own game.
There’s a bunch of dogs barking then they whine and stop so it’s assumed Amanda knocks them out with an electric taser she has. But it’s not constant abuse just one small moment for the sake of movie suspense
There's barking dogs being left alone in a dark room at a veterinarian's clinic. They also get scared by the bad guy, but there is no physical harm done.
Almost everyone in one of John’s traps is strapped down or restrained in some way. People also have foots and necks shackled and chains limit how much they can walk around. It looks very uncomfortable. One man is hung from a ceiling by his wrists, feet dangling above the ground, moaning in pain
A rape attempt of a prostitute is shown onscreen. Despite her assaulter not carrying out the act, it can still be difficult to watch if you are sensitive to rape.
Somebody cuts off their leg, someone gets their head cut off, someone has their hands cut, and someone cuts into their own scalp to get to their brain.
It’d take several paragraphs to describe it all, but it includes, among other body horror scenes (spoiler alert space, see below to read)
-a woman’s face is horrible seared under a hot lamp, flesh bubbles up and sizzled
-someone has to saw open their own skull in order to reach in and pull out a large chunk of their own brain, very graphic
-a woman has to saw off her own leg at the upper thigh with a wire, then stick a tube in to suck out the bone marrow (someone in my theater got up, picked up their stuff and walked out on the movie after the scene concluded). Very graphic.
-the flesh on part of a woman’s face is damaged by a deadly nerve gas, it almost looks rotted off
-someone’s eyes literally sucked out of their sockets by a strong vacuum
If you don’t want to see any body horror because it bothers/sickens you, I advise you to avoid the movie
Although two characters are pulled back by their necks and two more are hung up by their hands (one being a mid-credits scene), nobody is hung by their neck
There is a trap that involves the victim being waterboarded, only with blood instead. There’s no towel, it just is poured from a couple yards up so it goes up the persons nose and in their mouth. The person chokes and coughs and struggles to breathe through the entire thing. He survives, but waterboarding itself involves asphyxiation.
In an imaginary scene, a man breaks three fingers. A woman later has to cut off her leg and extract bone marrow, and another woman is forced to break her hand and foot.
Yes, and it’s done by someone whose eyes are attached to strong vacuum tubes that will remove them by suction. Which is somehow worse. Very graphic so if eye violence bothers you don’t watch the minute-long scene. The makeup effects are done so expertly it looks extremely realistic. And I have a strong stomach.
A major theme of the film is terminal cancer. The topic of parents dying and leaving behind children is discussed heavily. But no major characters who are parents die in the film.
He does die last however. However alot of Poc die in this movie. The first person to die is asian, then alot of mexican people die; 3 mexican. Only one caucasian person dies.
