It seems for most of the film that George and Martha have a son who is probably being quite severely abused. He is never seen, they don’t like to speak about him, George accuses Martha of “fiddling” with him and doing inappropriate things such as bathing him at the age of 16, and Martha says that he vomits from stress whenever he is in the same room as George. It turns out at the end that the son is imaginary, which was quite a relief
