His only friends are a couple of rats he raised at home, Ben and Socrates. Willard is very likely autistic based on his behaviour and mistreated, partially because of his autism. However this movie was made in the 1970s so it's not like they actually ever say that he's autistic. He's just seen as a weirdo in general.
