Fenrir does not die. He is immortal. Like most other gods of the Norse mythology, and superheroes in the genre, death has to be epic and is often temporary.
It is in the Norse lore that Fenrir actually consumes Odin during Ragnarok and would in turn be killed by Odin's son VÃÂðarr, but this does not happen in the movie.
So although peril happens to Fenrir, such as a great fall, he does not die from this.
The character is an adult when it happens but Hela is abandoned and banished by her father Odin. He was ashamed of the monster he created, as Hela was basically a killing machine.
Odin lies to Thor & Loki about his past including never telling them about the existence of their sister who he had imprisoned leading them to think that Asgards' history was far more glorious and honorable than it was. Thor believes he is the oldest and heir to the throne and also that when Odin cast him away to Earth it was the first time Odin ever did such a thing because of the lies.
The Grandmaster presents himself as a beneficent ruler even while sending captured beings to their death in gladiatorial games. He refuses to let his assistant use the word slave to refer to them even though no one can ever leave even if they survive the games. Even after there is a revolt he tries to convince the people that they should thank him for having something to revolt against.
Fenris is briefly shown dead in one scene before promptly getting resurrected. Additionally, Valkyrie momentarily falls onto a dead creature (not graphic, though some squishing/crunching sounds can be heard when she tries to get up again). There's also a dragon that dies towards the beginning and its decapitated head with green blood can be seen.
Not super graphic, Hela cuts out Thor's eye with her sword, but its not very detailed. You see the empty socket, but never anything with the eye itself.
Skurge, who had been Hela's ally until he regretted it, sacrifices himself to let the other Asgardians escape. He and his two guns fight off Hela's army so the refugee ship can leave.

No, but Odin was supposed to be in an old folks home (which can be triggering to some due to family in abusibe environments) and the characters forcibly strapped in their chairs can evoke the trigger
No, although it may be worth mentioning that there's a scene where Hulk repeatedly punches himself to stop himself from turning back into Banner. It doesn't cause any actual injuries to him, though.
