The worst thing that actually happens to Dorothy is that she is strapped to a table. However the medical treatment which they appear to have in mind would definitely be child abuse if they actually did it.

The so-called medical treatment which Dorothy is supposed to be getting would probably qualify as torture if it ever actually happened. It doesn't. I'm not sure why people have answered yes to this, there is no actual torture, the worst thing that happens to anyone is being hit by a lunch pail.
