SPOILER: The technicality of this is muddied. Isaac, controlled by the player, eventually is reunited by his girlfriend aboard the Ishimura, who appears occasionally to help for a while or offer advice only to strike off elsewhere, remaining in contact. It's not until near the very end of the game that it's revealed to both the player and Isaac that the girlfriend has actually been dead this entire time, and the apparitions of her seen may be the byproduct of an alien artifact present on the ship, which also was reported in audio and text logs scattered around the ship to cause hallucinations among the crew.

SPOILER: At the game's final stage, it's revealed that the love interest that brought the main character to arrive on the Ishimura was dead long before the events of the game, committing suicide to escape the horror overrunning the ship, in an extended version of a video transmission intended as a farewell message. We see her introduce an air embolism via injection into her arm, and she slumps dead on the spot in view of the camera.
