Many plasmids you use throughout the game alter your hands in disturbing and injurious ways (ie. blisters and sores on your skin, burns, ice shards through your palms and knuckles, etc)
(SPOILER) The player character is genetically programmed to obey someone anytime they say the phrase “Would you kindly.” A character manipulates them by constantly using this phrase to manipulate the player character throughout most of the game and even imprint them with false memories of their past.
There is a section of the Arcadia level that focuses on collecting enzymes from bees. This is a necessary part of the game and you can be attacked by them.
Before the first boss fight with Dr. Steinman. He is preforming a mutilating surgery on a patient which you see before he pulls out a Tommy gun to shoot the player.
Not so much squashed but a main antagonists head does get smashed in with a golf club during an cut scene. This scene cannot be skipped and will happen regardless of choices in the games.

Timmy in the Neptune's bounty level is tortured by Sullivan to get information on an antagonist. He is electrocuted several times which can be heard in an audio tape. You find his corpse being electrocuted with the power still on.
(SPOILERS) The bodies of a husband and wife who committed suicide can be found in their room. A nearby audio diary reveals they were distressed upon discovering their daughter had been captured and turned into a little sister Andrew Ryan is revealed to be the player character's father, shortly before his brutal death
Jumpscare spoiler: When you enter the Dentist’s office and approach the desk, there will be a cloud of smoke that appears all around you. When you turn around the dentist (thuggish splicer) will be right behind you. Total jumpscare.
not quite possession, but there are plasmids that allow you to change characters behaviours and have them do things for you. Also the main game mechanic allows the main character to be taken advantage of, similar to possession.
Several enemies have cancer due to genetic damage caused by the DNA modification serum-itself compared to a "good" cancer-used to justify magic throughout the game.
the first scene has the player enter rapture through an enclosed bathysphere, and upon arrival, is trapped inside as an enemy tries to break in to attack. the bathysphere is very confined and seemingly unescapable. (SPOILERS: the splicer is distracted and leaves you alone and unharmed)
The whole premise of the doctors backstory is he was performing surgery to “make freaks into something you could show in the daylight” and then he gains access to adam and his standards are set unreasonably high. There is also themes of eugenics, non consensual medical experimentation and so on
Depends on whether you rescue or harvest the Little Sisters. Rescuing results in the (canon) good ending, but harvesting even one results in the bad ending.
Large, fleshy blood splatters when enemies are hit, and a man being beaten to death until the weapon is sticking out of his skull. A woman is very graphically stabbed to death as well.
