The story logs of O-03-89 describes that it "breeds" employees that are sacrificed to or attacked by it. Reading these logs are optional and it does not get any more explicit than that.
Some abnormalities have covered mouths, and some abnormalities have the chance to give EGO gifts that will cover the mouths of your employees. For example, T-01-54 / Forsaken Murderer's gift is that of a belt wraped around the lower half of the face.
The abnormalities T-05-51 and F-01-37 reference the same story, the former also is based on self harm.
Carmen and Yesod are noted to have cut and scratched themselves to death respectively.
On a less serious note, Bald-Is-Awesome! forcefully shaves your employees. You cannot stop this.
The abnormality T-05-51 has a part resembling a scarred hand and its EGO gift consists of a blood-soaked bandage that covers an employee's hand and wrist.
Abnormality O-05-61 causes decapitation as a work gimmick, and Abnormality O-02-40 uses an attack that decapitates employees when it breaches. Also, as mentioned, Abnormality O-02-98 can make employees' heads blow up when work is performed on it.
Abnormality O-02-101/The Firebird burns people alive if it escapes. Employees and clerks killed by the firebird have a different death animation that reflects this. Abnormality F-01-02/Scorched Girl died in a fire as well.
There are various abnormalities that mutilate employee's eyes upon killing them (F-02-44, F-04-83, F-01-87, O-09-91, O-09-96, O-09-95, O-02-98, O-02-101, O-04-08, and T-04-06.) Spoiler warning: In addition, Malkuth's meltdown cutscene shows her bleeding from her eyes.
Spoiler warning: In his meltdown cutscene, Netzach mentions wanting to take enough EK to rust him, therefore killing him. He attempts to do this, but it fails.
Abnormality F-05-52/Open Can of WellCheers is based around a Korean urban legend about people being kidnapped and held captive on fishing boats after drinking juice that has been laced with sleeping pills.
The abnormality 0-05-47 (Don't Touch Me) can display a variety of jumpscares when interacted with— Blood dripping from the screen along with screams, the different warning levels flashing through the screen or the upside down lobotomy corp logo, right after it will forcefully close the game. It will also cause a voice to say "manjiji ma" (don't touch) in a rather frightening way. Additionally many other abnormalities have somewhat sudden screen effects, but these aren't really meant to be jumpscares.
mountain of smiling bodies/t-01-75 does. dunno if it counts as "someone" per this question but it throws up on people as an attack sometimes. also one of the abnormalities melts people into sickly brown sludge (in detail!) so uh
Abnormality T-01-54, while not in a mental institute, wears a straight jacket, and the small containment room he inhabits could very well make one think of a mental institution.
One abnormality is a murderer in a straitjacket who, according to lore, believes his head to be turning into metal to the point of banging it against walls
one of the major mechanics is quite literally called "sephirot meltdowns". employees can also panic once they reach 0 SP, causing them to have mental breakdowns.
(SPOILERS): Downplayed a bit, as the characters overall have a decent amount of distance from their pasts, but a lot of the characters are people who have died before and are struggling to cope with the events that led to their deaths.
The abnormalities O-05-47 and F-01-18 cause shakey camera movements.

Day 10 has it revealed that the cute art style of the game is actually a cognitive filter meant to keep your mind safe from the true horrors. That's about as far as it really gets.
the game has very blatant religious themes throughout the entire game with not only a lot of the abnormalities taking on christian themes but the entire base theming of the Sephirah being based on the jewish Sefirot from the Kabbalah, nearly all of the Sephirah's names are just blatantly ripped from the sefirot.

There is one character which shows severe symptoms of memory loss due to malfunctions as they have a mechanical body. This is a recurring theme which also affect's the story of the character's sister.
There are 3 possible bad endings, one good ending, and one true ending. The true ending is arguably quite sad, although not hopeless, as it leads into its sequel.
Abnormality F-01-57 uses a gun when she breaches or is hired; Abnormality F-01-69 visibly holds a gun and uses it, either when his Qliphoth Meter reaches zero, or when hired to do so. However, you aren't required to take either abnormality (unless you're going for 100%).
All of the clerk NPCs within the game are equipped with guns to defend themselves, many of the weapons you give your agents are guns (although you aren't required to use them), and you're able to summon a team of armored "Rabbits" with heavy artillery as a last resort (not required to use them either).
Basically, the only guns you're required to see are the clerks'.
