This is basically the definition of 1984. [Minor spoilers] An authoritarian government manipulating its people and constantly and consistent changing the past by altering and destroying physical evidence. Also, there's "thought crimes" which is a type of manipulation. The people are basically so brainwashed that they could be presented with actual physical evidence and deny reality.

Also, while it doesn't really count as soiling oneself, there's a scene where a character is described as using the bathroom very loudly. It's a little gross and might be worth mentioning.

No explicit racism by the author, but being written in the 40s, the book uses pretty outdated language that could be considered offensive (ex: “a Chinese”, “little yellow men”). This probably doesn’t count as hate speech but idk where else to put it. Also lots of xenophobia from the Party

There's several mentions of bombs being dropped. In one scene, Winston is walking right as a bomb has fallen, and finds the hand of a person who died in the explosion.
