Hope's Peak Academy is a special school, recognized by the government, that was established in order to gather and nurture top-class students who excel in various fields. This school has two courses: The main course, and the preparatory course. Anyone with the money can enter the preparatory course, even if they don't have any special skills. The 77th class of the main course is filled with students who not only have unique abilities, but unique personalities as well. However, the students learned that "Talent isn't the most important thing in life; it's to involve yourself with people and make yourself a better person while making memories." However, little did they know that a mysterious project and discrimination of the courses were happening right under their noses. Hajime Hinata, a boy from the preparatory class who craves for an ability. The fateful meeting he has drives the future of the school into an unimaginable direction.

Hajime Hinata's transformation into Izuru Kamakura is definitely this. Seiko Kimura's own transformation is also this. Mukuro Ikusaba could be considered an instance of this, seeing how it's mentioned that Junko Enoshima has always relentlessly abused her since they were kids, but they both collaborated in carrying out the Tragedy together.
[SPOILER] In the future side, No one was actually the attacker/traitor. Every death caused by the “traitor” was actually caused by a character being brainwashed by a video on a monitor and forced into committing suicide. So everyone was gaslighted by the mastermind into believing there was a traitor among them when there never was, essentially meaning they betrayed and killed each other over someone who never existed to begin with. Additionally, Chisa has been manipulating and gaslighting her boyfriend, Munakata, for years to gradually drive him to insanity. She’s also responsible for manipulating the mastermind into starting the killing game in Future side.
Sonosuke is constantly being drugged by Ruruka to be obsessed with her. She also tries to drug Juzo but fails. Seiko uses a ridiculous amount of some kind of performance enhancing drugs to become stronger. Nagito spikes judges foods with laxatives to cancel a test. Other uses of pharmaceutical drugs to heal or recover from poison or illness.
Makoto is tied up to prevent him from killing himself when he gets brainwashed, however he manages to get out of the ropes and comes dangerously close to killing himself.
In the Despair Arc, Teruteru threatens to sexually assault Hiyoko but is stopped before anything can happen. In the same arc a character sexually approaches another character who is visibly panicking and uncomfortable with the situation, but is stopped before any assault happens.
Mikan comes very dangerously close to raping a male character but is physically stopped by another character who just happened to walk in as she was about to begin sexually assaulting him.
[Spoilers] No cannibalism is shown on screen, but iit's heavily implied that Teruteru Hanamura engaged in cannibalism during his time as a SHSL Despair.
In Future episode 11, Makoto hallucinates Celeste’s death from the first game where she is burned alive. In the final Despair episode, a group of students kill themselves by running into a fire in a building.
A student is tortured with booby traps in a labyrinth until they are completely impaled by tons of spikes. They are covered in gore and *still conscious* for a short while afterward. Arguably the most disturbing and heartwrenching death in the franchise.
In the op for Future, every main character is shown dead In some way. Kyoko’s has her shown to have hung herself but this never actually happens in the show proper.
There are numerous in the despair arc in various different graphic forms. Absolutely don’t watch if you’re Squimish to torture. Chiaki’s death is basically her being tortured for 10 straight minutes
In the Future Arc, someone falls to their death in a very deep pit, the impact is not shown. In the Despair Arc, many people willingly jump to their deaths out of a building, this is gruesomely shown.
In Despair, 15 teens are forced to murder each other brutally in episode 7. A teenage girl has an extremely drawn out death in episode 10. Both scenes are extremely graphic and some of the most disturbing scenes in the entire series period.
In Future, a photo can be seen of a group dead children and a woman smiling above them after she killed them.
Teruteru mistakenly uses an aphrodisiac as an ingredient in one of his concoctions, causing his entire class and himself to become extremely horny. It's played for laughs but nearly causes one of the students to SA another student.
The main characters are kidnaped by Monokuma in future side. Ryota, Mikan and Chisa are all kidnaped by Junko and forced to work for her in Despair side.
Two dead characters are shown watching the events unfold in a theater. They talk as though they are in the afterlife. Another character's "ghost" appears to a character in the Hope Arc, but she disappears.
A half-brainwashed man says he’ll kill himself for Junko before slowly and graphically cutting his head off with a hacksaw screaming the whole time. The scene lasts for a full 10-15 seconds.
The whole Future arc takes place in a cramped building, which can be claustrophobic. A character in the Despair Arc is forced to make her way through a dangerous cramped hallway.
Literally several dozens of cases of main, supporting and background characters commiting suicide in various different graphic ways throughout both future and despair side. Absolutely do not watch if you are sensitive to suicide.
[Spoilers] Sakakura Juzo is outed very grossly, which might be triggering for some. He gets stabbed by his love interest, and cuts off his hand to take off his arm band. After this he bleeds out saving the others, but he was really trying to save Kyosuke.
The only black character in the future side is purely characterised as a gag by being a big buff black guy with an unfitting high pitched voice. While he’s not the first to die, he dies very early right at the start of episode 2 and purely just to demonstrate how the bracelets work
There is nothing explicitly stated but there are some questionable scenes between Junko and Mukuro (sisters). There is a scene where Junko puts her foot on Mukuro's b**t which makes Mukuro aroused.
In the Despair Arc, a whole class unknowingly eats an aphrodisiac drug that makes them all sexually aroused. This scene has many sexual references. See the sexual assault question for more.
In the Despair Arc, some high school students shoot each other to death in a classroom. A character points a gun at another character and threatens to shoot. A character is shot by a bullet but suffers no injury. The OP of the Future Arc shows two characters shooting themselves in the head (as a symbolic meaning). A character in the Future Arc is shot in the leg but survives.
