Lots of the village are gaslighted or emotionally gaslighted when coming to meet upon a more advanced society from the outside world. (Spoilers) They seem to worship a monster that makes you incapable of feeling emotion, and manipulates depressed villagers to worship as well as touch this monster because if they felt no emotion, they would not have to feel pain either. In result, many become cold heartless robots and monsters, a metaphorical fantasy display of how gaslighting abuse happens in the real world.

Hospitals are not always modern white concrete buildings. The hospitals here, while the best of what a more ancient tribe can do, still technically count as hopitals. (Spoilers) The people with abilities pay for this in return of a very short lifespan, so many die young around young adulthood or 30s at most.
