Shin Tsukimi, heavily implied to be a victim of abuse at the hands of his ex-friend Sou Hiyori, goes on to emotionally abuse and manipulate multiple characters in the game, namely 14-year-old Kanna Kizuchi.

while there is no confirmed violence, it is heavily implied there is a former abusive relationship between two characters along with an unbalanced power dynamic. the abused character expresses clear discomfort when questioned about the other character, and depending on which route is chosen, this character must encounter their abuser.

There is a certain amount of gore, the game is a death game. When a death happens, expect gore, but I wouldn't call it
excessive.

One character's (unavoidable) execution has a vice squeezing their waist. Another death has someone being crushed by enclosing walls; this one you can avoid seeing by not buying and viewing victim video 2.

one of the cards pulled in the death game forces the holder to choose between sacrificing themselves alone or killing the majority of the group to escape. inevitably this results in some self-sacrificing amongst the group. another character begs to have his life taken in the place of another character (the player chooses whether to allow this), and another character tries to sacrifice himself for a child (although neither of them end up dying at that point)

Occasionally in the main character’s hallucinations her friend’s appearance suddenly changes to become terrifying. There is also one early in the game where the appearance of a character’s sister will also suddenly change to become scary while the character screams in agony.

One of the characters is an ex-cop and refers to himself as "Mr. Policeman" or "your friendly policeman". While he idolized a cop, the game itself displays the police, and especially one officer, as corrupt in later chapters. The two cops that were displayed in a good light both resigned due to the issues.

one of the characters is implied in the spin off game (your turn to shine) to only eat warm soup with nothing in it as he’s uncomfortable with so many foods and textures. he’s underweight and physically weak as a result. could be reminiscent of certain eating disorders but it doesn’t really come up in the main game at all and isn’t a huge plot point

Characters kill themselves or contribute to their own deaths for various reasons: mostly to avoid torture or save things/people they care about. I won't list all of those occasions, since I don't think they resemble most IRL suicides, but there's also one character (a kid no less) toward the end of 2-2 who expresses feeling worthless/helpless before she tries to sacrifice herself. In an optional scene a kid also expresses suicidal ideation and dies. In terms of visuals, in 2-2 you see a guy holding a gun to his head and in 1-2 you see a guy stained with blood from slitting his own wrists. You don't actually see the wound in either case.

Nao (19) is heavily implied to be crushing on her much older, greying professor (though he never acknowledges it and wouldn't take advantage of her anyway). Keiji (a fairly mature adult) makes remarks that can be construed as flirty to Sara (17). In the side game, Kugie (16-17) is mentioned to have had an adult woman ask her out.
