Some characters die without getting the chance to say goodbye. In a non-standard game over in chapter 2, a character leaves the game without saying goodbye to the others. The latter is avoidable

while there is no confirmed violence, it is heavily implied there is a former abusive relationship between two characters along with an unbalanced power dynamic. the abused character expresses clear discomfort when questioned about the other character, and depending on which route is chosen, this character must encounter their abuser.

All characters are forced to wear a non-removable collar around their necks, and at points this collar is activated to kill them, meaning that their throats are often damaged. Most specifically in the first chapter a character’s head is burned off in this way in an unavoidable scene. Later depending on player choices a character may have seeds shot throughout their body via their neck/throat, which causes them a lot of pain

one of the cards pulled in the death game forces the holder to choose between sacrificing themselves alone or killing the majority of the group to escape. inevitably this results in some self-sacrificing amongst the group. another character begs to have his life taken in the place of another character (the player chooses whether to allow this), and another character tries to sacrifice himself for a child (although neither of them end up dying at that point)

One character is deeply traumatised from the start of the game by witnessing (and feeling responsible for) the recent death of her older sister. This shapes many of her actions and motivations for the duration of the game.

At least one other character will also experience the death of a sibling, and this is unavoidable, although player choices influence which sibling dies. Another character also unavoidably loses a long-lost sibling, although they only become aware that said character is their sibling in one route and after the death in question, so it’s possible to play through the game without knowing this.

One main character’s father shoots himself in front of everyone, although this occurs after said main character has already died himself and is unrelated.

The protagonist’s mother is unconscious on the floor at the start of the game - it is unclear as to whether or not she is alive.

Occasionally in the main character’s hallucinations her friend’s appearance suddenly changes to become terrifying. There is also one early in the game where the appearance of a character’s sister will also suddenly change to become scary while the character screams in agony.

One of the characters is an ex-cop and refers to himself as "Mr. Policeman" or "your friendly policeman". While he idolized a cop, the game itself displays the police, and especially one officer, as corrupt in later chapters. The two cops that were displayed in a good light both resigned due to the issues.

Characters kill themselves or contribute to their own deaths for various reasons: mostly to avoid torture or save things/people they care about. I won't list all of those occasions, since I don't think they resemble most IRL suicides, but there's also one character (a kid no less) toward the end of 2-2 who expresses feeling worthless/helpless before she tries to sacrifice herself. In an optional scene a kid also expresses suicidal ideation and dies. In terms of visuals, in 2-2 you see a guy holding a gun to his head and in 1-2 you see a guy stained with blood from slitting his own wrists. You don't actually see the wound in either case.

one of the characters is said in the spin off game (your turn to shine) to only eat warm soup with nothing in it as he’s uncomfortable with so many foods and textures. he’s underweight and physically weak as a result. could be reminiscent of certain eating disorders (e.g. ARFID) but it doesn’t really come up in the main game at all and isn’t a huge plot point

there is one male character who is wearing a skirt, but the only comments made about it are how he most likely stole it from someone who had died.

No one is misgendered, but a man character named Alice (who many interpret to be trans) gets teased about his gender

the character Kugie Kizuchi has been confirmed to be lesbian or bisexual by a side game and she dies before the events of the game but her death is frequently mentioned (though she is not killed for her orientation)

There is only one black person in the game and he dies at the beginning. I'm not one hundred percent sure of the order but I believe he dies at the same time as other characters.

Nao (19) is heavily implied to be crushing on her much older, greying professor (though he never acknowledges it and wouldn't take advantage of her anyway). Keiji (a fairly mature adult) makes remarks that can be construed as flirty to Sara (17). In the side game, Kugie (16-17) is mentioned to have had an adult woman ask her out.

There's an optional bathhouse scene, but the most you see is the top of the girls' chests; everything else is hidden by water.

Not really by the game. BUT if you buy Nao's info, there's an objectifying remark in it made presumably by a character in-universe. It's acknowledged as sexual harassment in the story.

Several characters die. Depending on the player’s choices, the second chapter may have a highly upsetting ending.

There is blood. The deaths are seen from a distance and the characters are pixelated, so it’s not too graphic in appearance.
