The dog fares relatively well compared to the other characters. It seems at one point that he might have been hit by a car, but it doesn't turn out to be true.
The killer, Stargher, was himself abused as a child by his father, with several sequences dealing with that fact. The movie seems critical of the idea that abuse alone can make a person evil, as it's implied the two protagonists also have similar dark backstories.
the child abuse is fairly graphic, it happens in the dreamscape and are the flashbacks of the main antagonist. His father is abusive to him and it shows at least three different scenes of abuse, including verbal and physical abuse.
The main character walks into a room with lots of clocks and there is a horse in the middle of the room after a clock gets to a curtain time there are glass sheets that fall from the ceiling and slice the horse into different sections then they move apart and show you the inside of the horse ( the organs are still functioning due to the horse only just being killed) it’s more weird and disturbing than scary and grotesque. The horse make a short and quick “neigh”, the ears also twitch and there is no blood

Not sure why anyone said no to this. A horse (albeit an imaginary one) is cut into sections while still alive. The horse does not technically die (you can see all the organs still functioning) or even appear to notice what has happened… but it’s still very graphic and disturbing.
There is a VERY sudden brief clip of a tarantula which happens near the end of the movie. It’s in the scene where she is dressed like a nun in all white. It happens right after J-Lo says “I can’t do that” and the dream-like border fades away. It’s gone after about 10 seconds.
Stargher's attacks are sexually motivated, as he masturbates over his victim's corpses while watching the footage of their drowning. We see representations of his victims within his mind, and they are presented in a sexually objectified way. There is also a scene showing Stargher's childhood, during which he appears to be forced to look at an unconscious woman's crotch by his abusive father.
there is a graphic torture scene of Vince Vaughn's character while he's restrained on a table. There is also ongoing torture of a woman in a room being slowly filled with water. There is a child abuse flashback scene where he is abused by his father that includes being burned by a clothing iron.
I'd say the worst part is at 1:17:40 - 1:19:18 when the serial killer pulls an intestine from a character. It's an important scene to the plot though so just listen to the audio if you can't watch.
Another gross part is nipples being ripped off at 1:31:20, lasts a few seconds
Otherwise it is no worse than your average R-rated horror movie
This weird made type of skizophrenia, which in the movie makes people kill and turn comatose, is often confused with autism in younger children, J-Lo explains 🙄
There are several scenes that suddenly blast noise or music. If you're sensitive to sound make sure you watch this with on something with dynamic audio compression controls.
The whole plot depends on some bizarre medical concuction that mixes up autism, skizophrenia and made up stuff to explain why the bad guy is bad and eventually in a coma. So diagnoses did it, basically
Not jokes, but androgyny is used as a horror element. There's one super muscular woman who attacks JLo, and there are dolls in the serial killer's house that follow a gender bending theme.
It is hinted that the bad guy who is doomed is gay or gender non conforming- in a flashback we see him getting abuse for being effeminate and playing with dolls.
Not technically but a father shows his little boy a woman’s crotch. She is not his mother but appears to be sleeping with the father. No actual incest but definitely some interfamilial sexual abuse.
Yes and no. A character who has been hurting others dies (and you learn complex nuances about them), but there are hopeful things at the end - including some lives that are saved/changed for the better.
the drowning is a slow torture that takes place over the course of the movie. There is also a flashback where a character describes being baptised and held under water and having a seizure. The camera angle for this scene flips upside down and goes under water, which could be upsetting for people with this trigger.
