The player character smuggles a pack of cigarettes prior to the start of the mission. These can be equipped in the secondary slot and are described as "hazardous to your health." Once equipped, they start draining the player character's health, but the smoke from the cigarettes can be used to reveal laser trip wires in certain locations.
There is a blast furnace that the player has to visit at one point during the game, and there are narrow ledges with machinery that the player character has to sidle across. If the player is not careful, the machinery running along the ledges will push the player character off, making them fall into a vat of molten metal where they are quickly burned alive.
An ally of the player character gets mind-controlled by one of the major bosses in the game, turning her gun on the player character until she's knocked unconscious before the boss fight begins proper. When that boss is low on health, he re-possesses your ally and attempts to make her commit suicide by gunshot.
Late into the game there is a torture sequence where the player has to mash a button to resist, or press a different button once to submit to the torturer; who then kills an ally of yours before the final escape should they submit. Depending on your actions here you will get a different New Game Plus reward, but failing to endure or submit to the torture will result in a Game Over that cannot be continued from, meaning you have to load your previous save.

At a few points in the game, including the start, there are bodies of water the player character can swim through. If the character's head is submerged, an O2 meter will be shown below the character's main health. If the O2 meter becomes emptied the player character's health meter will start draining rapidly until the character resurfaces or dies, resulting in a very graphic drowning animation.
