I'm not quite sure, since the ending left me super confused as to what was real and what wasn't. However, we see a desperate person doubting her own sanity and sense of reality due to what happens around her, including what people say and how they behave. Someone does call her crazy when she behaves in ways that are completely natural if what she's experiencing is real, but the thing is that they may in fact be right (if not using an appropriate word for it).

I'm not sure about the timestamps, but there are two scenes where it happens: First, when Kate enters the room where Miles is playing a guitar; He picks up a spider and handles it for a while before feeding it to a tarantula in a glass cage. Second, when Kate goes to sleep with the lamp on and Miles wakes her up; He has a spider in his hand that he claims he found crawling on her.

We're told that the doll mutilation was done by the child who owns them. In one of the very last scenes, a girl's porcelain doll falls to the floor, its face breaking into a few pieces. Characters with mental health issues are treated and spoken about poorly. The main character's mother is in a care facility and the housekeeper intercepts mail from her. The housekeeper then says that she hopes the mother's condition is not genetic and that you can't escape the inevitable (implying that the main character is mentally ill, which is implied multiple times by different characters).
