There are enemy animals such as wild dogs and bears that you fight and kill. There are also dead animals like horses killed in battles scattered around the landscape.
There are many kinds of dragon-like monsters, but they are all a) not technically dragons and b) strictly non-sapient. There's one quite funny quest where you're contracted to kill a 'dragon', it turns out to be basically an overgrown lizard, and if you correct the townspeople they don't give you your reward.
In the main game there are bug monsters which resemble crabs more than anything. The expansion pack Hearts of Stone however heavily features arachnomorphs. These are some of the most unpleasant giant spiders I have ever encountered in a video game, and if you are an arachnophobe I would give this expansion pack a miss.
There is an entire tree in the beginning of the game full of hanged people (callled hanged man's tree). Also the character named the Bloody Baron hangs himself in one of the possible endings of his quest
In an interrogation scene, a woman uses magic telekinesis to grab and squeeze a man’s groin. He yells in agony. This lasts for the characters to exchange a few lines of dialogue. The game gives the player an option to have her stop at this moment, and she will.
A main character is tortured/interrogated and her fingernails are removed. You do not see it happening, but you hear it and see some blood on her hand after the fact.
There is a character who was tortured and is an enemy during a Cat School Scavenger Hunt quest (Spoilers: The character is Kiyan/Mad Kiyan). This character is also potentially possessed although this is never confirmed. Also the process is which Witchers are created can be compared to torture (specifically the trial of the grasses). There are also other instances.
In one quest, multiple bodies are found who have been tortured. Their eyes were removed and coals were placed in the sockets while they were still alive. In another quest, a character removes his own eye. You do not see either of these things happen, but you see some damage to the eyes. There may also be other examples.
Mentioned many times, also implied. In the quest "Possession" in Skellige, you have the choice to throw a baby in an oven to trick a monster - the baby is unharmed, and nothing happens, but you can hear the crying for about a few seconds afterwards and it is very distressing
You will encounter orphans often, at least one of whom may tell you about the death of their family in detail if you ask them. You will also encounter children and young adults whose parents die after you have met them. You might be present for one of those deaths depending on your choices.
Showers don't exist, but there's an infamous bath scene where something humorously gross happens right at the start and a sauna scene with nudity later on.
While not really possession, there is a scene where a character is temporarily brought back from the dead using magic and forced to give information to the protagonists. It is quite disturbing but fortunately not shown for too long. Also, in the quest ‘Possession’ Udalryk is possessed by a Hym.
SPOILERS AHEAD
- Around the beggining of the game Geralt fights some people in an inn. At the end of the scene, you can see a man tu* from behind. You can hear the sounds but no v* is shown.
- Drunk NPCs tend to v*, especially during the night and in cities. They make r* sounds and sometimes the v* is shown, but it doesn't look realistic.
- Monsters called rotfiends make g* noises before they die. I think drowners make similar noises too.
- Uma v*s on Yennefer during the Trial of The Grasses. You can see the characters cleaning it up from the floor and off her dress afterwards.
Heart of Stone expanison:
- when you enter Shani's clinic/home, there will be ill NPCs v*ing.
- at the wedding there will be a lot of NPCs v*ing with (unrealistic) visuals shown for some reason, so be aware, mute the game when you're walking around if you want to.
- if you decide to give Shani alcohol during the wedding, she will v* during the... khm... boat scene.
There are multiple hospital scenes in the quest “Carnal Sins” including scenes in a morgue. This quest plays out like a murder mystery and multiple dead bodies are seen and examined. There are also religious themes in this quest (the murderer is killing people who question a certain faith).
In Hearts of Stone it is implied that Olgierd has self harmed. This is never seen on screen though. It’s a very subtle reference that is incredibly easy to miss.
Depending on your choices, there’s a prominent secondary character that commits suicide. SPOILERS ON HOW TO AVOID IT BELOW SPOILERS ON HOW TO AVOID IT BELOW SPOILERS ON HOW TO AVOID IT BELOW While progressing through the story in Velen, if you choose to free the spirit that was imprisoned in the tree by the crones, someone will commit suicide in a story quest not long after that (you can search for specifics if you want). If you don’t want that to happen, you need to kill the spirit in the tree.
There is an entire questline about it that is extremely graphic. It is impossible to avoid because it is a part of one of the main story arcs, the quest itself starts when you meet the Bloody Baron around the early parts of the game.
No, although the way some "races" (elves, dwarves) are treated sometimes resembles antisemitism and is possibly supposed to be an analogy for antisemitism.
Mislav, a gay hunter at the beginning of the game, mentions his dead lover depending of your choices.
Ciri, who's orientation is unconfirmed, but is attracted to women at the very least, can die depending on the player's choices.
Geralt is frequently spoken to in hateful ways because he is a Witcher. Common insults include; freak, mutant, and “Butcher of Blaviken” (the last one is derogatory only to Geralt because of a highly negative event in his past).
On one of the game's gwent cards, for the leader of the northern realms, the first card you get is Foltest's, which mentions that he was in a relationship with his sister.
There are enemies that are basically drowned bodies (they’re actually called Drowned Dead). There are also enemies called drowners and water hags that are very similar. It is also possible to die by drowning in the game.
