In Elite Agent Jin, a one-shot serialization, a character is seen to be punished by his sister for recklessly acting on his own. I don't think it would fall under 'abuse,' though, according to the numerous accounts or sources I've witnessed.

There are a couple rads that appear maybe twice. They don't stalk anyone, but they do appear from, say, underground or from a defeated trion soldier, simply to collect analytical data for safekeeping; such as the strength of the combatants who defeated the trion soldier, or where to spawn a gate.

1- in the manga, a character gets his hand cut off. But in the anime his wrist is just impaled.
2- During battles, characters often loose limbs in their trion bodies. Trion bodies are NOT their real bodies, when they loose their limbs it's not gory/bloody in the slightest. We see cubes (trion) floating away from the wound. They always have their limbs in their normal body, and their trion body gets the limbs back

Hyrein's Alektor is able to send projectiles in the shape of various spineless creatures that turn the trion (from trion bodies) it comes into contact with, back into trion (cubes.) So during the Afto Invasion arc, one will see a couple of misshapen, glowing hands or feet. I wouldn't call it mutilation, though.

1- the city is shown in ruins when they talk about the backstory of the city. This may include car wrecks
2-Yuma is hit by a car twice in episodes one and two, however he and the car passengers are completely fine both times. He looks hurt the 2nd time, but his face immediately regenerates and he hops right back up as if nothing happened
