In the last episode, in the very end of the anime, we see what appears to be Akira's torso and head by the side of Ryo, his companion and fellow "main" character. His legs and arms have been decapitated and he is very much dead.
In the very first episode when they’re at the club the one girl is starting to get killed by a devil from the inside or something and right before it happens it looks like she’s peeing but I’m assuming it’s diarrhea. It is onscreen, graphic, and there is audio.

Ryo is seen in a very modern and fancy hospital after the Black Sabbath, wherein it's explicitly stated to be a hospital, even if it looks nothing like one aside from some base elements.
There are a few minor characters, one man running around naked gets hit by several cars, another that could be interpreted as mentally ill is shouting bible quotes and then pulls out a gun and falls from a higher place. Several people fall from high buildings.
Yes, episode 8 almost exactly 5 minutes in. it shows the people on top of a building and then cuts to miki’s dad so look away after it shows her dad. very brief maybe 10 seconds but the whole last few episodes i’m pretty sure there’s a good bit of this stuff
If i’m not wrong, one minor character says he’ll kill himself after being possessed by a demon, and I think Akira says he’ll do the same if he loses control, but I could be wrong
Multiple people who are complete strangers die in the first few episodes, though none are explicitly stated or shown to be pregnant in any way. Multiple women die, though, so please keep that in mind when viewing this anime.
Several characters who are LGBT (some canonically and some heavily implied to be canonical) die, but without spoiling the show, it is not problematic in the context in the slightest bit (no, they are not portrayed as villainous either). However, it may still be upsetting to some people.
Very specifically, there is a scene in episode 7 where Miki is harassed by some cops on the street who call her foreigner for her green eyes- note that Miki is a half- American Japanese citizen. A flashback follows where very young children tease and bully Miki for the color of her eyes.
