There’s a storyline told mostly through holotapes that strongly implies that a boy is being sexually abused by a man. I didn’t finish the mission because of this, but from what I understand it may turn out that’s not really what was happening.

You can come across dead radstags that have been hung to dry out, in flatwoods there is a static dead brahmin, and between the overseer's camp and flatwood you can come across a dead radstag yearling which is implied to have been killed by the wild mongrals near the corpse
if you use molatov cocktails, it is possible for an enemy to burn to death if they are low enough on health and hit directly leaving a charred body behind.
Two missions come to mind where you can get stuck in a cave and it seems to close in on you. The missions are “a colossal problem“ and “lode bearing.”
In “a colossal problem,“ it is implied that the player has to escape the cave before it collapses, but in reality nothing happens if the player stays in the cave. The mission is still passed.
In “lode bearing,“ if the player fails to escape the cave in time, the player dies and their loot is lost in the cave.
There is a clown costume, and you can get a miscellaneous quest to kill a wendigo while in a clown costume - it used to be a possible daily challenge before Legendary Run was added, i haven't see it used as one since then but it is possible it still it
There are many mannequins in stores and other various buildings. You can also have mannequins in your camp to display outfits. This means that other players may have mannequins in their camps as well.
One of the locations you visit when following the overseer's journey is an old mental insitution. Some rooms still have hospital beds in. There is a wing where you can find some feral ghouls some of which are inside of the empty rooms.
There is a camp item of a traffic light which flashes red, yellow then green, there is also a camp item called a stobe light which when powered flashes. The player vender item also has a slight flashing light, you can avoid seeing it while browsing by looking downwards and away from the light before interacting with the item, you may still see a slight flashing from it.
in an early game quest at morgantown airport, you find out a character called Miguel who was in a relationship with a man named Garry, but as originally there were no npcs in the game this is told through terminal enteries and notes you can find.
There is a perk called bloody mess which when equipped can cause bodies to explode when killed. If you get a headshot at close enough range, the head can explode. Areas occupied by supermutants have blood bags, and when enemy bodies despawn but still have loot they become a meat pile which is red.
