A young woman is molested by her uncle and becomes pregnant. The woman's fate is determinant, but she can be saved and the uncle can be murdered by the player.
A few sidequests do feature people who claim to have been harmed by their spouses and a main companion discloses his father was physically abusive to both him and his mother.
A major plot point is that all the babies are being born without souls and are called "Hollowborn". It is mentioned that many of the children are murdered, or have the souls of animals placed into them, which can drive them crazy. Also, a young boy can be found dead and can be avenged.
During a side quest an npc will ask the player to abduct a baby for their plan. The player can refuse or trick this character. It's entirely possible to complete the sidequest without interacting with the child.
In a way. There is a magical curse where babies are being born without a soul. Some "hollowborn" children are killed after, and in a village, the parents are killed as well. Parents are fearful of the curse and it can be devastating when the child is discovered to be hollowborn.
If a character fails a skill check, the outcome might say that they sprained or broke a limb. Though, all it does is impede fighting and nothing is shown.
During the white march dlc a companion or the player must stay behind to activate a mechanism that will not give them time to escape.
Their sacrifice can be avoided with a specific companion or the correct equipment.
Enemies can explode in bloody bits upon death, though I think it can be turned off in the settings. Also, there are several bloody locations, pools and pits of blood, intestines and limbs, and just about every location has a murder cult.
A companion says several times that he was abused by his father and neglected by his mother. It's all in past tense, but it is very relevant to his character.
There is an asylum where people who see visions from their past life are sent, in hopes of curing themselves. These patients are tortured and experimented on. Many quests are at this asylum, including a major plot point where, depending, all the patients are killed.
I would highly caution anyone with DID or OSDD to be careful/take care of themselves if they choose to play this game, as a lot of the stuff with awakenings can hit very close to the ableist misconceptions systems may face (such as certain souls/alters being dangerous, systems driving the host insane, etc)
