I would say the main storyline so far is that the travelers sibling missing, and once they end up finding them the sibling leaves again with not much of a explanation.
In Fontaine's main story, Lyney tells the traveler how he and Lynette were adopted by a wealthy man who then betrayed them by parading Lyney around and giving Lynette away to another man.
It could be argued that Arlecchino becomes the abuser when she takes on the role that her previous foster mother had had due to the fact that that orphanage is technically raising children to be military. However, she is not nearly as bad as her mother was. I just figured it should be mentioned just in case.

There are several characters who drink a lot and talk about drinking a lot who can definitely be seen as alcoholic. The game doesn't seem to see this as a problem, so none of the negative effects of alcohol abuse are shown.

Durin, a corrupted dragon, was killed off-screen several hundred years ago. His bones appear prominently in the geography of the Dragonspine region, and you can make weapons out of his remains as well.
There are sometimes spiders in ruins. However, they're not very scary. They don't move from their location, die easily, and disappear along with their web. They are also rarely seen.
A Playable character from Liyue has a talking snake that sits around his neck, Inazuma has a giant dead snake skeleton and it will be shown alive during brief animated flashback cutscene, and Sumeru has enemies and hostile critters that are snakes.
Mualani rides a shark-like object in her attacks, though it is cartoonish and cute-looking. There are also aquatic saurians (dragon-ish things), but those aren't technically sharks
butterflies, fireflies, and fictional butterfly like bugs called "crystalflies" frequently exist in the enviroment, and most of them are necessary. an update recently added huge beetles all over the islands of inazuma. they're very scary.
In the main story quest in Fontaine, it's mentioned that Lynette was "given" to a man, but she was saved before being harmed. They then found other girls locked in his basement. All of this is spoken about, not shown in any way.
There is a playable character who wears something akin to a face mask but it’s purely a stylistic design choice; she is shown taking it off in her idle animation as well
in the sumeru archon quest one character offers her arm to someone as part of a deal, but it never ends up getting amputated, it's just mentioned that it could happen
There are cutscenes where the player character becomes unconscious. Besides that, the characters you're playing as can "die", but can easily be revived so it makes more sense to think of them as unconscious.
teeth are not damaged in game, however several giant broken teeth from a large serpent are seen in Inazuma, and you can collect fangs for upgrading weapons in Mondstadt
Playable characters have torture mentioned in their character stories (ie Rosaria) but there isn't anything shown on screen aside from that one time you torture an Abyss Mage. There's no gore, but a lot of screaming.
While the game does have a lot of killing and stuff, there is no blood when you kill a character or enemy, and enemies are turned to ash and disappear when killed.
The Tsurumi Island questline involves a kid being sacrificed along with 2 other kids implied to have been slaughtered. A playable character's backstory also includes a child dying. One playable character is a dead child that’s been revived as a Jiangshi. Enkanomiya's lore involves the puppet rulers, the Sunchildren, being killed after they reached a certain age. Hu Tao's story quest, Chongyun's hangout, the "Of the Land Amidst Monoliths" main quest, and the "Book in the Woods" world quest all involves ghosts of dead children.
I'm the chasm quest line, (spoilers) Xiao attempts to sacrifice himself so the others can get out safely, but is saved by Zhongli before anything can happen.
No playable character has died as of writing (and all playable characters are mostly human or human-like), but there are non-human characters from certain quests that do die like in Nahida's second story quest, the Aranara questline, and in the Akitsu Kimodameshi event (human-like character).
In the new fontaine quest there is a thing called “water from the primordial sea” which makes one feel euphoric and has implications of drugs in my opinion. But if someone from fontaine has too much they dissolve into water, which happens in a cutscene to a npc.
it’s implied that two npc women date the same man ( also an npc ), but i don’t think it’s actually confirmed, and as well doesn’t affect playable characters.
There are ghosts in some quests, and they tend to be children. They are not particularly scary or malicious. Hu Tao is often pictured with ghosts, but they are highly stylised to be cute and do not resemble people. Wuwang hill is implied to be haunted, and there are ghostly orbs (hitodama) in the area.
In Zhongli’s story quest act 2, it was revealed that a character was possessed. In Yae Miko's story quest, you willingly allow a youkai to possess you. And throughout Sumeru's main questline, one character does possess you and some NPCs, additionally, you also possess some NPCs.
In Bennet's side story, it is implied that his cooking makes him (or another adventurer, depending on your choices) sick, but it is not graphic or onscreen.
There are police explicitly mentioned in Inazuma, and Mondstadt and Liyue both have their own governing bodies (the Knights of Favonius and the Millelith,) but they are generally reflected in a neutral light.
In Mondstadt, the Holy Lyre is destroyed (by the god who owned it in the first place and he replaces it with a fake). In Liyue, the Jade Chamber and its replacement are sacrificed to defeat antagonists and fall to the bottom of the ocean, along with everything inside them. There is a later quest in which you go retrieve some of these items.
One playable character named Sigewienne wields a giant syringe for her Burst, however there’s no injections depicted and she just uses the syringe to shoot waves of hearts.
I would say that Scaramouche has implications of mental illness(es) though never confirmed. And he is one of the main villains in the sumeru archon quests.
Layla seems to have Dissociative identity disorder. Freminet has an idle animation that could be seen as him dissociating. In Sumeru a Character experiences their own past in the third person, but in an intentional and non distressing way. In several areas, but particularly in Sumeru, some magical effects make discerning reality difficult for characters.
Layla appears to have DID. She has clearly defined trigger (excessive stress) and her second personality matches the "protector" type personality that actual people with DID describe happening. She is in no way treated as dangerous because of this/she foes not have a "violent" version of herself.
several characters have autistic traits. The character "Sucrose" seems very heavily autistic coded, and is frequently mocked and insulted by her coworker Timeaus for her traits like infodumping and hyperfocusing. She is frequently treated as being "weird"
kind of. in the 3.3 update, a character with suicidal ideation attempts to completely erase himself from history (nongraphically and through fictional means), he succeeds in altering the past so that he never existed as no one remembers him, but he is still alive in the end.
Certain areas, quests and dungeons, both playable and referenced in lore, follow unusual rules of reality. For example, time may flow abnormally, or certain spaces may be inverted.
thoma has a very audible chewing idle animation. I do not have misophonia but I don't use him bc of this, as it annoys me bc it is not a pleasant sound
In the 2.1 update's archon quest, the main character goes through a moment of great distress after witnessing a death. This moment is enhanced by the soundtrack and visuals to emulate some sort of dissociation, which felt extremely uncomfortable to me.
There's a character, Ganyu, who is implied to have an eating disorder, she mentions very often denying herself food and and trying to keep her weight low, it could be very triggering for someone with an eating disorder.
suicide is occasionally mentioned. a recent fishing event features a full readable suicide note. while it isn't a common feature, this game clearly is not afraid of mentioning it, and you should be wary if it's a severe trigger for you.
As of the 3.0 update, a character named Collei has a deep PTSD response to Paimon suddenly touching her. Collei is known to have a very traumatic past as well, and frequently shows signs of her PTSD.

The character Cadence is based on the Kandakes of Kush, and likely Amanirenas specifically. Kush was in what is now Southern Egypt, but the people there were dark-skinned, and Cadence appears middle eastern.
What do you mean there are only two characters of color? I agree that only two of them are dark skinned, but they have two entire nations based on China and Japan respectively, so most characters are actually Asian. In fact, if you consider all current 41 playable characters, 21 of them are POC. I do however advise that people don't have high expectations for anything beyond good lightskin Asian representation
Religion in the game is quite distinct from real religion, because gods are present front and center in the plot. The different ways each region or group of people respect their god(s) are discussed several times. Several elements of the game reference or take inspiration from real-life religion. For example, in Mondstadt there is a cathedral, and in Sumeru there is a valley in a desert named Wadi al-Majuj.
many of the women in the game have extremely sexualized and revealing outfits, in a way that's fairly standard for anime. I would not say there was actual sexual content outside of character's with revealing outfits generally acting flirty.
Beidou was homeless as a child and says this in her hangout event. Other characters also reference it sometimes. || Razor could be classified as homeless, although he considers Wolvendom to be his home, where he lives with wolves. || Kaedehara Kazuha is a wandering samurai whose family lost everything — but again, he spends most of his time on Beidou's ship and basically lives on it. || Venti, aka the God of Anemo/Barbatos, doesn't seem to live anywhere. This doesn't appear to bother him though because again, he is a god. || Aether/Lumine is technically homeless until a quest after Liyue's Archon Quest, in which Madame Ping gives them a teapot holding a pocket dimension to live in.
The Thundering Fury artifact set's goblet lore states that it was used to store the blood of innocents as a offering to the Thunderbird. You learn that one of those sacrificed was a child.
