A scary severed head pops out of a box during Fright of Their Lives in the first act.

Beetlejuice quickly pops up from an elevator in the orchestra pit during the end of the first act.

A very large Beetlejuice head prop and hands props come onstage. It is not insanely rapid, but absolutely unexpected. This is during the same scene as the elevator one. This can be frightening for audiences in the middle orchestra.

A large sandworm puppet is brought onto stage multiple times throughout the show. It is not rapid, but can be startling.

Beetlejuice jokes with the audience multiple times. He speaks to the audience directly often, and will make a few jokes and point to an audience member (sat in stage-right-side front orchestra) and say “this guy knows what I’m talking about.”
