can't speak for the manga but in the anime the city gets infected with herpes after engaging in (presumably) consensual sex, however the herpes itself is depicted as a monster with tentacles that says its lonely and attacks and restains the while blood cells while forcibly removing their clothes and in some cases licking them all without their consent

i would argue that in the anime the designs for the white blood cells are very fan-service-y, with their uniforms being open at their chests leaving their breasts mostly uncovered which could be understandably very off-putting to some viewers especially since the previous anime "cells at work!" had no problems making the uniforms fit the white blood cells correctly when they were all men
