There is a scene where the villain appears to talk to himself using two different personality in a way that is extremely similar to common misrepresentations of D.I.D. The main character even accuses him of having D.I.D. However, it is later revealed that the second personality was his twin brother that was living inside his chest
Especially gory scenes include: A woman getting stabbed in the eye, a man getting shot through the mouth by an arrow, a girl getting impaled on multiple metal rods, a woman getting drag by a metal wire around her neck and snapping her neck, a woman getting the side of her face split open by a giant blade and a mutated fetus bursting out of a woman's chest. Multiple sequences from the 10-30 minute mark feature strobes. One particular kill has a strobe going on for most of it. Shots from the killer's perspective also have jittery movements that might be hard for the eye
Spoilers: Of sorts? Toward the end, a man's brother, who possesses his body, is "birthed" from said body as a slimy, bloody fetus. The same happens to the main character, Nami, at the end, the fetus emerging from her chest.
