When a follower dissents you are expected to put them in prison and "re-educate" them until they correct themselves. This can be seen as the Lamb (an abuser) forcing their victims to forgive them to be freed.
It is TECHNICALLY possible for the player to set up a scenario where they are married to a follower, and then murder that follower. If familial violence is included, it is stated that one character wounded all of his siblings in the past, and these wounds seem to persist.

One of the Four Bishops, Leshy, had his eyes torn out by The One Who Waits. While covered by a blindfold, his eye sockets are constantly bleeding. Also, when preforming Rituals, the Lamb's eyes will start to bleed. (Edit: Thanks CobaltCosmos!) [[SPOILERS]] The final boss has multiple eyes, which pop out of his skull and are used to attack the player. One is permanently damaged and he's shown without it in the end game.
While most lesser monsters simply turn into skeletons, the major 4 heretics could be a bit gory, and the surroundings in the areas are rather disturbing. The lamb also appears very beaten up when reduced to 1 heart.
