the creatures in episode 4 do a form of this repeatedly against the guards.
in episode 6, right at the beginning when the meeting is interrupted, there's a lot of brutal violence around people's heads. the same is true towards the end of the same episode when they're trying to escape. the one woman who tries to help scanlan gets it pretty graphically and it's up close.
to a lesser extent, a few people also (LIGHT DESCRIPTION, NOT QUITE A HEAD SQUASH) get their heads/faced punched in, not as much graphic visuals here.
in episode 5 there is the hanging that the other commenter mentioned happening in the original campaign, i don't have the timestamps but it is right near the end, i'd guess the last 2-3 ish minutes
episode 5 is really graphic (just in general, as the whole show is). when the guards outside the keep get ambushed and the blond one disappears off screen for a moment, look away for a good 10-30 seconds.
Thousands of people die due to various acts of fantasy violence. Dragons destroy most of a kingdom in season two and people are shown being burned, squashed, melted, poisoned, and otherwise killed by the dragons. There are also roving bands of orcs, vampires, and other monsters that kill people on screen.
A father succumbs to poison/acidic gas in front of his wife and children. In another shot, a mother and child are seen being engulfed by the same gas, although no further shots of them can be seen.

the creatures that delilah summons in ep 4 are a form of ghostly creature. they share a lot of qualities with ghosts, ghouls, and zombies.
this is also the case in episode 5, but they're different creatures altogether. they're much more like zombies than ghosts.
During the song "Making my Way," Scanlan, Pike, and Grog are shown trying to navigate a river. At one point Grog falls into the river and is used as a boat.
Keyleth’s ears ring from worry in episode one. Percy is heavily implied to have serve PTSD from the Briarwoods’ attack and his torture at their hands. In episode three this is shown extensively in the latter half.
Most fights have flashing lights from attacks. Percy's gunfire tends to be flashing, sometimes in rapid succession. There are also flashing effects from spells, magic arrows, and some elemental attacks.
there aren't any jokes at the expense of trans people, and no jokes about men in dresses.
there is one moment in episode 5 where scanlan briefly turns into a tall and busty woman, but he looks identical to himself otherwise--he comments that he looks really good, and grog is "aroused and confused" (mostly bc it's scanlan, not because he's a man).

In the original show the character is alive for both the arc that was adapted for season 1 and the arc that will be adapted for the greenlighted season two, so no reason to spoil something that might never be made into the animated show. Also is important to point out their death has nothing to do with their sexuality, it has to do with the extremely violent and dangerous world (as the other triggers point out) they live in

The dragon Raishan is shown to have a curse that behaves very similar to a terminal illness. The curse will eventually kill her, and she speaks about looking for a cure but finding none.
this show is *very* gory. it's animated, but it's very, very explicit and graphic, basically in every episode.
i, personally, can't stand gore in live action shows, but rarely do i have to look away from the violence here, even when it's gruesome. that's totally up to personal tolerance, though.
