Seven years after the death of his wife, company executive Aoyama is invited to sit in on auditions for an actress. Leafing through the resumés in advance, his eye is caught by Yamazaki Asami, a striking young woman with ballet training.

Yes, a lot. Two characters have their fingers removed (both off-screen). One character's (now healed) hand is shown, this happens as the main character is having a dream about being at Asami's house (just after collapsing in his house). This is quite a drawn-out scene and there are multiple shots of his hand up-close.
You never see needle go into eyeball, but you see a POV of her inserting needles near eyes and it is sloooow and the sound is gross lol.
From then to the end you see many many shots of him with his eyes closed and needles sticking out of his eyelids and under eye area. He eventually opens them but still had needles above & below. It is intense & visceral but you do not see needle into eyeball. Hope this helps!
Depends on your tolerance for gore and your definition of “excessive” but she does decapitate someone and amputate many body parts with what appears to be a sort of piano wire. A lot of it is off screen and in my opinion they aren’t gratuitous with it so it’s really your own tolerance of that particular act and the sounds more than anything.
I'm surprised this section isn't loaded with comments and warnings, because it gets worse than mentioned below: after the woman v***** - which she does into a dog bowl - she makes the man eat it.
To cushion that, however, the also below-mentioned comment about it being real has been proven false (not calling out the person who commented for not telling the truth, it IS on imdb, but I did some digging because it bothered me so much, lol). On the audio commentary, the director makes this statement and then laughs it off as being a joke about how far people go with method acting.
It's extremely brief, but in the montage of auditioning women, we're shown one woman pointing to her wrist and describing scars there. She goes on to mention that she needed to be institutionalized because of her self harm. It's very short, a "blink and you'll miss it" clip, but it is there.
No but shots of Asami's very thin body with bones protruding are shown constantly throughout the film. This would be very triggering to anyone with eating disorders.
Sort of. The movie's kind of vague about it, but (spoilers) the main female character is revealed to have been abused by her stepfather, and in scenes of the abuse it has extremely sexual overtones. Also, her stepfather shown masturbating to her at one point, though it is very briefly.
There is a scene which occurs in the main character's head where he imagines all the women in his life attempting to give him oral sex. The creepy ballet instructor is also seen briefly masturbating in a kind of flashback/hallucination, presumably in front of a child. He physically abuses her, which has sexual undertones. It is also implied that two main characters consentually have sex off-screen.
Another "blink and you'll miss it" scene, during the main character's hallucinations/dream sequence, he sees a vision of his son getting shot in the head. It isn't real, and it only takes a few seconds for the scene to play out, but it is there.
